The main quest "For Whom the Bell Tolls" kicks off immediately after you finish "Wedding Crashers." You find yourself imprisoned in Trosky, with Hans facing execution once the bell tolls 12 times. Your first task isn't optional: you must haul sacks to a shed. Pick up a sack from the wagon and carry it to the shed adjacent to the fenced area. Drop it inside, then repeat until the objective is complete.

Next, you're directed to assist the blacksmith, who reluctantly agrees to let you craft a horseshoe. Head to the forge through the archway opposite the blacksmith. Return to the noble's kitchen and ascend the stairs, avoiding guards by staying crouched. Distract any guards with a pebble if necessary, then sneak up to Thomas' room where Adele is caring for him. Hand over the Fever Tonicum to Adele, triggering the quest-ending cutscene.
